We are currently seeking a highly motivated Clinical Pharmacist to join our team and make a positive impact on patient care and health outcomes.
Key Responsibilities:
- Review and interpret prescriptions to ensure accuracy and suitability for patients.
- Provide pharmaceutical care by monitoring drug therapies and preventing adverse drug reactions.
- Counsel patients on medication usage, dosage, side effects, and storage.
- Collaborate with healthcare providers to optimize patient treatment plans.
- Monitor patient progress and adjust medication as necessary.
- Ensure compliance with healthcare regulations and pharmaceutical laws.
- Maintain accurate records of patient care and medication inventory.
- Provide a consistent process of patient care that ensures the appropriateness, effectiveness, and safety of the patient’s medication use.
