Job Description

Home care nurse provide care to patients in their homes under the guidance of a physician. They perform regular visits where they monitor the patient’s condition, assess their wounds, and change dressings as required. Home care nurses also write reports and communicate with the doctor after each visit.

Responsibilities:

Provide care in coordination with the physician and other health care workers

Assess the patient’s condition, health progress and needs

Manage patient as per scope of services

Identify care issues and make recommendations about changes to the patient care plan as per consultant recommendation

Communicate with family caregivers, listen to their concerns and update them on the patient condition

Keep records for future reference

Maintain the documentation of care provided accurately in EMR

Ensure that patient privacy and confidentiality is duly protected at all times

Treat patients and their family with courtesy, respect, and caring behavior

Take immediate action to resolve any issues, complaints or distress expressed in any way by the patient or their family within her limits and escalate to higher level

Assess the patient for risk of developing pressure ulcer and implement pressure ulcer preventive measures

Administer medication to the patient as per the doctor’s prescription and document the same in EMR

Ensure all clinical consents are uploaded in patient EMR

Work as a liaison between the patient and treating physician if the patient is registered to RFH

Escalate the patient’s condition to treating physician if any

To communicate the observations, non-compliance, issues and challenges compiled from all possible information sources towards compliance to quality standards

Compile and summarize data for presentation at monthly departmental meetings and quarterly meetings as requested

Work closely with nursing manager, Head of the department and doctors to ensure a high standard of nursing care

Generate regular reports regarding the adherence to quality standards, achievements and possibility of improvement, and forwards copy of line manager

Promote use of evidence-based practice

Ensure readiness of home care Kit at all times

Ensure the compliance in completing the process of home care services

Perform any other duties as directed by the line manager

Any other responsibility as assigned by the ward manager
