Job Description
Key Responsibilities:
1. Medication Dispensing:

Accurately dispense medications as per the prescriptions provided by healthcare providers.

Ensure the correct drug is dispensed in the appropriate dosage form, strength, and quantity.

Review medication orders for accuracy, appropriateness, and potential interactions before dispensing.

Label medications with instructions for use and provide information regarding side effects, potential drug interactions, and proper usage.
2. Clinical Consultation and Advice:

Advise and collaborate with healthcare professionals (doctors, nurses, and other hospital staff) on the safe and effective use of medications.

Provide information about drug interactions, side effects, dosages, and administration techniques to healthcare providers.

Assess and monitor patients’ medication regimens to ensure optimal therapy and therapeutic outcomes.
-
Participate in rounds with medical teams, providing expertise on pharmaceutical care for individual patients.
3. Patient Education and Counseling:

Counsel patients on the safe and proper use of prescribed medications, including information on dosage, timing, side effects, and precautions.

Answer patient queries regarding medication regimens, over-the-counter products, and any concerns they may have about their treatments.

Educate patients about the importance of adhering to prescribed therapies and follow-up care.
4. Pharmaceutical Care and Medication Management:

Monitor patient responses to medications, and make recommendations for changes in treatment plans when necessary.

Review patient medical histories and laboratory results to identify any risks associated with prescribed medications.

Implement and monitor patient-specific medication therapy management plans, particularly for patients with chronic conditions or polypharmacy needs.

Oversee medication storage conditions and ensure that medications are stored in compliance with regulatory standards.
5. Pharmacy Operations and Inventory Management:

Maintain an accurate inventory of medications, ensuring that stock levels are adequate and medications are reordered as necessary.

Ensure medications are stored under proper conditions to maintain their efficacy and safety.

Manage medication stock to avoid expiration, and dispose of unused medications according to hospital policies and regulations.

Participate in audits and quality control checks to ensure compliance with hospital and regulatory standards for medication handling and storage.
6. Drug Therapy Monitoring and Adverse Event Reporting:

Regularly monitor and document patient progress regarding their drug therapy, identifying any adverse effects or side effects.

Report adverse drug reactions (ADRs) and medication errors as per hospital protocols to ensure patient safety and continuous improvement.

Work with the medical team to make adjustments to treatment plans in response to adverse reactions or changing patient conditions.
7. Regulatory Compliance:

Ensure adherence to hospital and pharmacy-related regulations, including federal and state pharmacy laws and ethical guidelines.

Maintain proper documentation for controlled substances and other medications with special regulations.

Keep updated with the latest pharmaceutical practices, laws, and medical developments to ensure compliance and the highest standards of care.
8. Research and Continuing Education:

Stay updated with the latest research, clinical trials, and developments in pharmacology and therapeutics.

Participate in training and professional development to continually improve pharmaceutical practices and clinical knowledge.

Assist with hospital-based research, providing guidance on medication management for clinical trials or new drug therapies.
